Nepal has more registered nurses and health workers every year, yet public hospitals still have unfilled posts and many trained staff leave for better pay abroad. Teaching and tutoring are steady, but private school pay is often low and rules for consultancies and preparation classes tightened in 2026. Almost every job here needs a council registration or a licence, so the first step is always to study at an affiliated institution and pass the right council or commission exam.
Research as of 2026-10. Fees, rules and loan terms change often. Items marked Check rest on secondary sources, so confirm them with the office or on its website before you pay or sign anything.
The market today
Health demand in Nepal is growing faster than the public system can hire. A Medical Education Commission projection reported in 2026 says Nepal will need about 301,895 health workers by fiscal year 2030/31, leaving a gap of around 63,226. Nepal Nursing Council figures on its own website show about 129,420 people registered in total, but studies suggest only a fraction (one count says about 45,000 nurses) are actively working in Nepal, because many move abroad or leave nursing. Private hospitals, clinics, labs and pharmacies keep opening, but pay in many private hospitals has been below the government minimum, which fuels migration. In 2025 a nurses' pay dispute ended with large private hospitals agreeing to match government pay and smaller ones to pay 80 percent for now. On the education side, community schools are government funded while private schools, preschools, tuition and coaching centres are a large private market. In 2026 the cabinet approved new rules for educational consultancies, language and preparation classes, which add cash deposits and licence conditions. Care work for elderly people at home is an emerging field in Nepal, and foreign care-worker schemes (for example Israel) draw large numbers of applicants but have been stuck on fee disputes and fake-certificate problems.

Nepal Nursing Council website shows total registrations of 129,420 (88,583 nurses, 37,625 ANMs, 2,090 specialists, 858 foreign nurses or midwives, 264 midwifery graduates), last updated 27 September 2026. Registered does not mean working in Nepal.nnc.org.np (opens in a new tab)
In December 2024 the Health Ministry set a minimum monthly pay of NPR 34,730 for nurses and health workers (equal to a fifth-level government employee). In November 2025 a deal said large private hospitals and medical colleges pay at government level and other hospitals 80 percent of it for now. Real pay in some private hospitals had been NPR 15,000 to 20,000.kathmandupost.com (opens in a new tab)Check

Medical Education Commission fee ceiling for B.Sc. Nursing is NPR 10,36,000 (10.36 lakh) in total; the figure is the same across several years and appears in the 2083 (2026) fee notice. It is a cap, not what every college charges.blog.educatenepal.com (opens in a new tab)Check
Cabinet approved the Educational Consultancy, Language and Preparation Classes (Operation and Management) Regulations in July 2026. Education Act reports say teachers at the same level in private and public schools should get equal pay set by government, but monitoring is weak and private schools often pay less. What drives demand
Public hospitals have thousands of sanctioned but unfilled posts, and the health workforce gap is projected to remain through 2030/31.
Rising numbers of private hospitals, polyclinics, diagnostic labs and pharmacies in towns and along highways.
Ageing families and migration of young adults leave more elderly people needing home care, though this market is still informal.
Foreign demand for Nepali nurses (UK government agreement) and caregivers (Israel scheme) pulls trained staff out of Nepal and keeps local pay pressure on.
Families keep paying for private schools, preschools, tuition, language and exam preparation classes.
Government pay and allowance rises in 2026 improve the public sector offer for teachers and health workers.
Challenges
Private hospital pay is often below the government minimum, and the November 2025 pay deal was only partly enforced as far as reports show.
Heavy workload and burnout among nurses push people out of the profession.
Teaching posts in community schools are limited and filled only through Teachers Service Commission exams, which have been delayed at times.
Private school and preschool pay is low and rules are weakly enforced.
Preschool, tuition and coaching rules are still being rewritten, so requirements can change.
Many colleges are not properly affiliated, and fake certificates are a known problem in the health and care sectors.
Where in Nepal
Kathmandu Valley, Pokhara, Chitwan, Butwal and Biratnagar have the most private hospitals, colleges, labs and coaching centres, so the most private-sector jobs and the most competition.
Provincial towns and remote districts in Karnali and Sudurpaschim have the most unfilled public health posts, so health assistants, ANMs and lab staff are in short supply there.
Border and highway towns suit clinics, pharmacies and diagnostic centres because patients travel there for care.
Kathmandu, Lalitpur, Bhaktapur and large towns have the biggest preschool, tuition and language class markets.
Business and work ideas
Retail pharmacy (drug shop)
medium risk
Sell medicines under a registered pharmacy. The law requires a qualified pharmacist or pharmacy assistant on staff, a Department of Drug Administration (DDA) registration, and premises that meet storage and dispensing standards.
Skills
D.Pharm or B.Pharm with Nepal Pharmacy Council registration, Stock and expiry management, Customer counselling, Basic accounts and VAT
Suits
Pharmacy diploma or degree holders with some savings, or a pharmacist partnering with an investor.
Note
No official start-up figure was found, so none is given. Cost depends heavily on rent, stock and location; get quotes from local wholesalers. Renewal is handled through provincial offices, and lapsed registrations are cancelled.
Small clinic or polyclinic
high risk
A private clinic or polyclinic run by registered doctors or health professionals. Needs health institution registration under the Public Health Service Act and the private health institution standards, plus inspection.
Skills
Medical qualification and council registration, Staff management, Compliance and record keeping
Suits
Registered doctors, health assistants (within their scope), or investors partnering with them.
Note
No sourced cost. Standards depend on the type and bed count; local governments can register smaller facilities. Ask the municipal health section and the province health ministry which level applies.
Diagnostic laboratory or sample collection centre
high risk
Pathology or diagnostic lab. Needs registered laboratory technologists, equipment meeting the national laboratory guideline, and approval before opening.
Skills
Medical lab technology diploma or B.Sc. MLT, Registration with Nepal Health Professional Council, Quality control and reporting
Suits
Lab technicians and technologists with experience, ideally with a doctor or pathologist partner.
Note
Equipment is the main cost; no figure is sourced. A small sample collection centre linked to a bigger lab is cheaper to start.
Home care and elderly care service
medium risk
Send trained caregivers or nurses to look after older, sick or disabled people at home, or run a small day-care or elder-care home for families.
Skills
Nursing or caregiver training, First aid, Patience and communication, Basic business management
Suits
ANMs, nurses and trained caregivers who want to work for themselves, especially in Kathmandu Valley and other cities.
Note
No dedicated licence for home care agencies was found; register a business and check with the local government and the province health ministry whether your service needs health institution registration. Never advertise nursing services without registered nurses.
Physiotherapy and rehabilitation centre
medium risk
Clinic offering physiotherapy for sports injuries, back pain, stroke recovery and children. Therapists must be registered with the Nepal Health Professional Council.
Skills
BPT or diploma physiotherapy, NHPC registration, Exercise therapy and equipment use
Suits
Registered physiotherapists, alone or with a doctor referral partner.
Note
No source was found for how to register a physiotherapy centre itself, so ask the local government health section and the province health ministry. No cost figure is sourced.
Preschool, kindergarten or Montessori
medium risk
Early childhood centre for ages 3 to 5. Reports say Montessori and preschools register at the local level (ward or municipality), not at the district education office.
Skills
Montessori or early childhood training, Child safety, Parent communication
Suits
Teachers or parents who want to run a small school close to home.
Note
Current official registration checklist was not found; the 2014 news report on ward-level registration may be out of date. Ask the municipality education section for the written requirements. No cost figure is sourced.
Tuition or coaching centre
low risk
After-school tuition for school grades, entrance and licence exam preparation (for example Teachers Service Commission, nursing council or Loksewa preparation).
Skills
Strong subject knowledge, Teaching ability, Marketing to parents and students
Suits
Teachers and graduates with a strong subject, working evenings and weekends first.
Note
The 2026 regulations cover language and preparation classes, so check registration and deposit conditions before opening a formal centre. Small home tuition needs little money but also builds no brand.
Language institute (English, Japanese, Korean, German and others)
medium risk
Teach language classes for study or work abroad. Falls under the 2026 Educational Consultancy, Language and Preparation Classes regulations.
Skills
Language certification such as IELTS, JLPT, TOPIK or Goethe, Teaching skills, Course design
Suits
Certified language teachers and returnees from Japan, Korea or Europe.
Note
Press reports on the new rules mention licence, renewal, premises and reporting conditions, and a cash deposit for consultancies. Whether the same deposit applies to a pure language class was not confirmed; verify with the Ministry of Education.
Skill training institute (care, health aide, hospitality, technical)
medium risk
Short-course training centre offering caregiver, health aide, and similar skills. Courses should be affiliated or approved by CTEVT so the certificates are recognised.
Skills
Trainer qualifications for the course, Equipment and practice space, Placement links with employers
Suits
Experienced practitioners and teachers who can meet CTEVT standards.
Note
CTEVT published a short-course affiliation notice in May 2026, but its requirements and fees could not be read. Download it from ctevt.org.np or call CTEVT before investing. Fake caregiver certificates caused rejections in the 2025 Israel scheme, so only issue real, recognised training.

PCL or B.Sc. Nursing and Nepal Nursing Council registration, then hospital application
Pay per month
NPR Reported range NPR 15,000 to 20,000 before the 2025 deal; hospital offers of about 24,287 to 29,587; deal says large hospitals should now match government pay and others pay 80 percent
Check that a college and each programme are really affiliated to CTEVT or a university before paying any fee; unaffiliated certificates are not accepted for council registration.
Never buy a fake training or caregiver certificate. In the 2025 Israel caregiver round the Department of Foreign Employment rejected 563 applications as fake and about 150 selected people had fake documents.
Do not pay an agent a large fee for a nursing or caregiver job abroad. The government set low fee limits in the past and agents have charged far more; use only licensed agencies and government schemes, and check fees on the Department of Foreign Employment site.
A council exam fee is small. Anyone asking for a large sum to guarantee a pass or a licence is cheating you.
Private hospital pay can be much lower than the government minimum; ask for the written salary and hours before accepting an offer.
Do not open a pharmacy, clinic or lab before the registration is issued, and do not run it without a registered pharmacist, nurse or technologist on site.
Fee figures on college listing sites can be out of date. Get the fee in writing and compare it with the Medical Education Commission ceiling or CTEVT notice.
The ANM programme status is unclear: one source says CTEVT stopped offering it from 2020 while colleges still list it. Ask CTEVT whether your course counts.
Rules for preschools, coaching and consultancies are changing in 2026. Ask the municipality or ministry for the current written requirements before investing.
Medical certificates from clinics that do not examine you properly have caused workers to fail destination country health tests; use only approved clinics.
